Pages come out crooked and upside down on their own - not operator error! Why does this happen and can it be fixed?

I had the same problem scanning to PDFs with the CanoScan Toolbox until I went to the File Settings dialog, and unchecked the option "Detect the Orientation of Text Documents and Rotate Images".   Now it seems to work more predictably (no more scans that have been rotated for no apparent reason).
